Please bring a photo ID, insurance card, medication list, relevant records, and completed forms when requested. Arrive early enough to complete registration.

Insurance & Billing

Coverage varies by plan. Call your insurer and our office before the visit to confirm network status, referral requirements, copays, and covered services.

Prescription Refills

Use the patient portal or call the office. Allow adequate processing time. Some medicines require an appointment, monitoring, or an in-person evaluation. Do not use the appointment form for urgent refill needs.

Medical Records

Contact the office for release instructions. Written authorization and identity verification may be required.

Office Policies

Please provide advance notice when rescheduling or canceling. Policies may apply to late arrivals, missed visits, forms, and after-hours requests.
